Help with a Federal Agency

If you can't get an answer from a federal agency in a timely fashion, or if you feel you have been treated unfairly, our office may be able to help resolve a problem or get you the information you need. While we cannot guarantee you a favorable outcome, we will do our best to help you receive a fair and timely response to your problem.

Residents of the 7th Congressional District of New Jersey can contact us for assistance in dealing with Federal agencies. In order to better serve you, this form will generate a printable pdf document that you should sign and mail, fax, or deliver in person to our office.

Guidance for Submitting Casework Requests

Please include all pertinent information and claim numbers in your correspondence—such as:

  • Your Social Security number for a case involving Social Security;
  • VA claim number for a case with Department of Veterans Affairs;
  • Taxpayer identification number (Social Security number, if individual) for an Internal Revenue Service problem, etc.;
  • Your address, home phone number and daytime phone number (if different than home) so that we can obtain any additional information from you that might be necessary;
  • Copies of any related documents or correspondence that you may have from the agency involved;

For Passports:

Please be sure to attach your proof of travel (confirmed flight itinerary). 

Please include your passport locator number. Also, the date you applied and if you paid for expedited service.

If you are completing this privacy release form for a minor child, please be sure to provide the child’s name, the child’s social security number, and the child’s passport locator number. 

For USCIS:

Please do not provide any Social Security Numbers. USCIS will not accept any form with a Social Security number included.  Please provide all information for both the petitioner and beneficiary (in some cases the petitioner and the beneficiary may be the same person, that is acceptable), including A number, receipt number, form number and place of birth.  If you have received a receipt from the USCIS for a petition that has been filed, please upload the receipt. The receipt will help us direct our inquiry - on your behalf - to the correct office within the USCIS.    

For Department of State  (For visitors’ VISAs):

If you are completing this form for someone who wishes to visit the United States, please include the proposed visitor’s name, date of birth, place of birth.  Please indicate if the proposed visitor has ever been denied a visa to the United States.  Please indicate if the proposed visitor has ever overstayed a visa while in the United States. Please indicate what ties the proposed visitor has to his/her home country.    

For Social Security Disability:

If you are completing this form for a child with a disability, please include the child’s name, the child’s date of birth, and the child’s Social Security number.

Before submitting this form, please carefully check your submission for accuracy and completeness.  Inaccurate and incomplete information will hinder and delay our ability to provide you with the most accurate and prompt response. 

  • Information about Another Individual

You cannot request information about another individual, unless the individual is a minor and you are that individual’s custodial parent, or the individual is an adult, and you have a Power of Attorney over the individual.  If you have a Power of Attorney over the individual, please upload a copy of the Power of Attorney.
